Your privacy is very important! During your first appointment, you will be presented with waivers to sign. A Medical History Form, a Treatment Consent form, and a Photo Release Form. We do not disclose your personal information or share it with other outside entities unless otherwise authorized by you.

Your information is used for internal statistics, marketing, or educational purposes. We do not send spam emails. We only communicate with our clients and potential clients regarding new services, price changes, special offers, and appointment notifications.

COVID Guidelines

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. The state of medical knowledge is evolving, but the virus is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air.

People reportedly can be infected and show no symptoms and therefore spread the disease. The exact methods of spread and contraction are unknown, and there is no known treatment, cure, or vaccine for COVID-19. Evidence has shown that COVID-19 can cause serious and potentially life threatening illness and even death.

Pure Body & Aesthetics cannot prevent you [or your child(ren)] from becoming exposed to, contracting, or spreading COVID-19 while utilizing Pure Body & Aesthetics services or premises. It is not possible to prevent against the presence of the disease. Therefore, if you choose to utilize Pure Body & Aesthetics’ services and/or enter onto Land Mark premises you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

Clients are required to wear a mask or face covering at ALL times. Children, young family members and extra guests are not allowed and cannot accompany you during appointment.

Health Conditions

If you are currently under medical supervision for any conditions, it is recommended that you consult your physician before booking any of our sessions. Body treatments are not advised for pregnant women.
